Have you accessed the Post Covid Assessment Service recently?

The Post Covid Assessment Service provides a predominantly therapy-based approach to help patients with self-management strategies for symptoms and is available via referral from a GP or hospital.

A survey has been set-up to capture the views of patients who have accessed the service, with feedback being used to help make improvements to the way the service is delivered in the future.
